Transcript Requests

Transcript requests must be made in writing and contain your signature. For your convenience you may click on the link Transcript Request Form to access a form to print and send to the office. You may mail the request, fax the request, or bring the request in person. If you do not use the Transcript Request Form, the request must include:

• your name
• social security number
• date of last attendance
• the number of transcripts requested (indicate # of official or unofficial transcripts)
• address where transcripts should be sent
• daytime phone number where you can be reached
• your signature

There is usually a one- to two-day turnaround on transcript requests. You should allow additional time for requests made during peak periods (that is, registration and grade processing). This is a free service. The fax number is 240-895-4449.
