The document also includes the link to the <a href=\"https:\/\/docs.google.com\/forms\/d\/e\/1FAIpQLSeGUKW0Oy6_A9Qh3uG7rzKwHzLtE5Sh9Sltcs-OfSLUIJOfvw\/viewform?usp=header\" target=\"_blank\" rel=\"noopener\">Google Form<\/a> where you will upload your completed proposal as a <strong>PDF<\/strong>.<\/p>\n<h3>Transcript (Submitted by the Student)<\/h3>\n<p>Submit an unofficial transcript with your application.<\/p>\n<h3>Mentor Recommendation (Submitted by the Faculty Mentor)<\/h3>\n<p>Instead of a traditional letter of recommendation, mentors complete the <a href=\"https:\/\/docs.google.com\/document\/d\/1r-_XbKPWhyYe5O4UiHwJit4BIYy8Ut_d\/edit\" target=\"_blank\" rel=\"noopener\">Mentor Recommendation Sheet<\/a>.
