In order to keep you and our campus community healthy we follow Maryland State and CDC guidelines regarding Immunizations and Screening for TB and require current, accurate Health information.
Whether you are an Incoming Freshman or a Transfer Student, below is required information you need to complete/submit:
We do not accept outside medical forms/records in place of the forms below. The below forms/information MUST be completed and submitted in order to be accepted. Please note the Immunization Form has to be signed by a health care provider.

- Insurance Card Front and Back
- Only used if we have to send out labs or if you fill a prescription
- SMCM Student ID Card
- Demographic and Consent Form
- Students who will be under 18 when they start college MUST have this form signed by a parent/guardian.
- Health History Form
- Immunization Form
- Exemption Form (if stated on Immunization Form)
- Insurance Card Front and Back
Please complete and submit ALL Health Forms/Documentation before coming to campus.
This can be done through the
SMCM MediCat Health Records.
To sign in, use your SMCM email and password.
Deadline: August 1 for the fall semester and December 15 for the spring semester
We no longer accept forms mailed or faxed to the Wellness Center.
Be aware that this information must be completed in order for you to move on campus or to register for classes otherwise a hold will be placed in your file until these forms are received.
It is the students’ responsibility to ensure and double-check all information is complete and accurate. Parent or legal guardian signature required if student is under 18.
St. Mary’s is not liable for any medical information omitted from these documents. It is advisable to keep a second copy of all information for your own records.
All information shared with the Wellness Center is confidential and will not be released without your consent.
NEED MEDICAL INSURANCE?
Students are not required to purchase health insurance, nor do we endorse any specific health insurance programs for students.
You can contact the St. Mary’s County Health Department at 301-475-4330 or visit their website about how to obtain health insurance if uninsured. This is general information regarding applying for state insurance and the information that will need to be provided.
