New Student Information

To help keep our campus community as healthy as possible, we follow best practices and recommendations from the Maryland Department of Health, the CDC, and other public health experts regarding immunizations and tuberculosis screening.

All students are required to complete the forms below and upload the front and back of their health insurance card, along with a state-issued ID. Please submit only our official immunization form, completed, signed, and stamped by a licensed medical provider. Do not upload other immunization records.

Please submit ALL completed forms before coming to campus.
We do not want check-in with Residence Life to be delayed when you arrive!
