Passport Fees

Money Orders / Personal Checks made payable to US Department of State

Adults:

• Books – $110.00
• Cards – $30.00

US Postal Service Fees

• $25.00 Execution Fee

Passport photos are available on site – $15.00

Please Note: Payments for Execution Fee & Photo’s will need to be in the form of a check or money order made payable to U S Postal Service)

Identification

You must bring these two forms of identification:

• Certified Birth Certificate (Include official seal)
• Current Driver’s License or State Identification Card

Or

• Naturalization Certificate (If naturalized Citizen)
• Current Driver’s License or State Identification Card

And

• A photocopy showing the front and back of the applicant’s ID made on plain white, 8½ x 11’’ standard paper.

Photocopies must contain images on only one-side of each page (not on the front and the back).

If copies cannot be made on the same side of one page, the applicant may present two separate pages; one displaying the front of the ID and the second displaying the back of the ID.

All applicants must appear in person, including minors.
