What do I need to bring to my medical appointment?
Bring a state-issued photo ID, health insurance card if you have it, and arrive 10 minutes prior to complete any necessary paperwork. If you have a sports physical appointment, you must bring in your completed and printed 8-page sports health history packet in order to be evaluated by the nurse practitioner.
How do I schedule a sports physical?
You must be registered on SportsWare through the athletic department before scheduling your appointment at the Wellness Center. Reach out to your coach or athletic trainer for more information on SportsWare. Once you are registered, complete and print the 8-page sports health history packet – you will need to bring it to your appointment in order to be evaluated by the nurse practitioner. Will I need to travel off campus if I need a prescription or lab draw?
All services rendered from the Health Services team are free. We have several prescription medications on-site, so it is unlikely that you will need to travel off campus if the nurse practitioner writes a prescription for you. If the nurse practitioner orders blood work, or you have a lab order from an outside provider, we can draw your labs with no additional fee. Depending on your insurance policy, you may incur a fee from the processing Laboratory.
How can I get my prescriptions if I can’t get off campus?
There is a prescription delivery service on campus in partnership with The Pharmacy at PJ Bean. Students can stop by the office to pick up the instructions or (follow this link) to set up an account.
